The clinic was founded in 2006 by community leaders and in 2010 the clinic was designated as an FQHC-LAL (Federally Qualified Health Center, Look-alike).
It is the Mission of Knox-Winamac Community Health Centers to provide quality, primary health care services, accessible to persons in our community including the medically underinsured and uninsured.
